Over the past few months I have had a record high number of patients testing positive for either chronic/recurrent Epstein-Barr virus (EBV) or acute mononucleosis. I’m not sure what’s in the air – maybe people are more lovey-dovey than in the past – but viruses are on my mind!
EBV is a virus that is spread by intimate contact – not necessarily sexual. Mononucleosis (or mono for short) is often called “the kissing disease” for this reason. It can however also be spread by sharing utensils. The virus inserts itself into immune cells known as B-lymphocytes and basically hijacks replication and then circulates freely and widely in the bloodstream. Your immune system fights back by inducing cell death in these B-lymphocytes. This process causes (in people over the age of 5) the symptoms of mononucleosis: profound fatigue, swollen glands, and sore throat which can persist for varying amounts of time depending on the person. EBV is also found nearly everywhere, and in nearly everyone but certainly isn’t always problematic. In fact if you were infected early on in your life (before age 5) the virus is usually acquired without these symptoms.
